The State Party has released the Surrogate Affidavit form for use by people unable to attend the March 26th presidential precinct caucuses in Washington due to illness, disability, military service, work schedule, or religious observance.
Surrogate Affidavit forms are available online from the State Party website (the link is in the first paragraph) and are also available from the Spokane County Democrats office, which is located at 1403 W Third Ave (at Cedar) in downtown Spokane (phone number is 509-324-8525).
The form must be completed, signed, and returned to the Seattle office of the State Party so that it is received by Friday, March 18, 2016 at 5:00 pm.
