Because Surrogate Affidavit forms have to be received by the State Party in Seattle on Friday by 5:00 pm, be sure to scan it and email it back before the deadline --- don't count on the mail to get it there by Friday.
Once you've completed the form in its entirety, sign it and then send it back!

Although faxing the form to the State Party is a listed option on the form, the volume of faxes being sent to the fax number makes it very likely that your fax will not go through.

The Surrogate Affidavit form process is to be used by people who are unable to attend the March 26 caucuses because of (1) illness, (2) disability, (3) work schedule, (4) religious observance, or (5) military service.
If you qualify to use the form, the paperwork must be received by 5:00 pm at the office of the Washington State Democratic Party in Seattle on Friday, March 18, 2016.
A timely submitted form permits your presidential preference to be included in the results of your precinct caucus on March 26, 2016.
